IDENTITY OF THE RESPONSIBLE PARTY


In accordance with the Federal Law on the Protection of Personal Data Held by Private Parties and pursuant to Article 15 of this Law, Dr. Ernesto Martinez de la Maza, with address at Camino a Sta. Teresa 1055, Hospital Ángeles del Pedregal, Torre Ángeles, Office 920, Colonia Héroes de Padierna, CP 10700, Magdalena Contreras Delegation in Mexico City, Federal District, is responsible for collecting the personal data of our clients and for the confidentiality, use and protection thereof.


PERSONAL DATA


Dr. Ernesto Martínez de la Maza guarantees that the client's personal information is protected in accordance with the Federal Law on the Protection of Personal Data Held by Private Parties and will be used solely for identification purposes in any type of legal relationship and to provide the services requested by the patient. For the aforementioned purposes, we require the following personal data: full name, address, occupation, marital status, income, and asset information.


PURPOSE AND PROCESSING OF PERSONAL DATA


The data collected by Dr. Ernesto Martínez de la Maza may be used to obtain the necessary information from patients for the medical treatment they entrust to him. For the purpose stated in this Privacy Notice, we will collect the patient's personal data when provided to us through: electronic, audio, visual, and written means. Likewise, the personal data obtained from the patient will be used for:

  1. Provision of medical-hospital services, including, without limitation, (i) consultations, (ii) hospitalization, (iii) surgery, (iv) diagnostic studies, (v) and other purposes related to health services.

  4. Studies, records, statistics and analysis of health information.
  5. Record keeping for service monitoring, future service provision and generally to monitor any contractual relationship.

  7. Communication of relevant information and services.

Dr. Ernesto Martínez de la Maza will collect and process sensitive data related to health status, medical history and background, lifestyle information, and other data necessary or convenient for the purposes stated above. This sensitive personal data will be maintained and processed with strict security and confidentiality for purposes related to the provision of health services and in accordance with this privacy notice and applicable laws, regulations, and standards.


TRANSFER OF PERSONAL DATA TO THIRD PARTIES


Dr. Ernesto Martínez de la Maza may transfer, within and outside the country, the personal data in his possession to other Doctors, Hospitals, Clinics, Study and Research Institutions, Laboratories, Insurance Companies and third parties subcontracted for purposes related to those indicated in this privacy notice.


RIGHT TO WITHDRAW CONSENT FOR THE PROCESSING OF PERSONAL DATA


The patient has the right to access their personal data and be aware of the treatment being given to them, rectify and cancel their personal information for justified reasons, as well as oppose the processing of the same or revoke the consent for such purpose that has been granted, by submitting their request in writing, specifying the reason and addressed to the legal representative of Doctor Ernesto Martínez de la Maza, to the email address ernesto@drmartinezdelamaza.com.


CHANGES TO THE PRIVACY NOTICE


Dr. Ernesto Martínez de la Maza will communicate any modifications made to the Privacy Notice via email, which will be sent to the patient.
